Hello parents and carers,

Further to my recent message on the Messageboard, you are welcome to place your orders for Videos from last year. Please complete the order form in full and make it clear which video you would like to order.

If ordering more than one video, please complete a different order for each different video (though you can order more than one copy of any given video on a single order form.)

We are happy to take your order in good faith - though for obvious reasons we will be unable to release the completed videos until payment is made.

When sending completed payment into school (cheques made payable to St. Anne's Catholic Primary School, please - or cash) please clearly mark:

your name (as included on the order)
your child's name and
the class your child is currently in.

If you are a Year 7 parents with no more children in St. Anne's School, (presumably ordering the Year 6 production from last year) please clearly indicate your name and address and a current phone number - so we can get the completed order to you.

All videos are £5 each. The videos will be standard DVD video quality.
